• e-mail address;
• preferred method of communication;
• information about skydiving experience;
• number of jumps;
• home drop zone;
• type of parachute system;
• information about the date of the reserve parachute repack;
• information about self-packing of the parachute;
• information about skydiving disciplines;
• height;
• weight;
• information about previous jumps;
• information regarding the need for helicopter transportation;
• other information voluntarily provided by me in the relevant form.

Information concerning health status, diseases, medical contraindications, and other special categories of personal data is not requested under this consent.

I consent to the following actions being performed with my personal data:

• collection;
• recording;
• systematization;
• accumulation;
• storage;
• clarification;
• extraction;
• use;
• transfer in cases provided for by law;
• anonymization;
• deletion;
• destruction.

Processing may be carried out both with and without the use of automated systems.

Personal data may be processed using information systems and services used by the Operator to operate the Website and process requests, including Tilda CRM.

This consent remains valid until the purposes of processing the personal data have been achieved or until the consent is withdrawn, unless otherwise provided by the legislation of the Russian Federation.
After the purposes of processing have been achieved, the personal data shall be deleted unless there are other lawful grounds for their storage and processing.
